We are pleased you have chosen Mid-America Driver Training for the Juvenile Driver Improvement Program.  This class is a requirement to have your driver’s license reinstated.

Because we offer this program once per month, pre-registration is required. Your completed registration form and non-refundable payment of $100.00 is due one week prior to the scheduled class date. Cash, personal check, MasterCard/Visa or money order is accepted. Failure to do so will forfeit your participation.   

Plan to bring a form of identification to be copied for our records. (This can be a social security card, court papers or a state ID)

Mid-America Driver Training reserves the right to cancel.  You will be notified by phone if your class has been postponed. All payments intended for the original date will be applied to the next scheduled class.  

 Your session is scheduled for Saturday, ___________200_______.  

The class is held from 8:00 a.m. until 2:30 p.m.  You may bring snacks or lunch with you.  You may also want to bring additional money to purchase snacks here or to order lunch from a local restaurant.
