Mid-America Driver Training looks forward to providing each student with an exciting and enjoyable driver education experience.   Enclosed you will find the school contract.  Please read and complete both sides of this form.  Signatures from both student and parent are required.

 

When completing the school contract, please note the following:

1.)    The student’s name on the school contract should read exactly as it does on the permit.

2.)    The permit number is the License No.  This is found on the front of the permit under the student's name and address.

3.)    The issue date/validation date is at the top to the right of the student's picture. (This date is the same.)

 

Cost of the entire course is $340.00.  Personal check, (made payable to Mid-America Driver Training) cash or credit card (Visa/MasterCard) may be used for payment.  Any returned check will be subject to a $25.00 service charge.  Payment options are as follows:

1.)    Pay $340.00 the first class session

2.)    Pay $170.00 the first class session and $170.00 the first driving lesson

 

Classroom is completed first, after which driving will be scheduled. Our office will contact each student to arrange the driving schedule.  Each student will drive four times, two hours each lesson. These may be scheduled at your convenience.  A waiting period between class and driving is common especially in the summer and early fall months. The amount of time will fluctuate, depending on our schedule.  If the student is not able to attend a scheduled driving lesson, our office must be contacted at least one hour prior to the lesson.  Students will need to bring their I. D. card and permit paper when driving.  Failure to notify and/or not having a permit will authorize Mid-America Driver Training to add a missed driving appointment fee of $25.00 to the student’s account.  Those students who live outside of Bowling Green city limits will be responsible for their transportation to and from driving lessons.

Students are expected to complete the entire course within six months of the starting date.  Those extending past this six-month deadline may be subject to an additional fee.

Plan to arrive 15 minutes early for the first class session.  Food and drinks from home are permitted or you may purchase items during class breaks.

ATTENTION PARENTS:

Mid-America Driver Training will be showing the updated version of the classic driver education film “Signal 30”. The Ohio Department of Public Safety produces this video.   It runs approximately 19 minutes and deals with actual accident scenes and victims.  The new version is quite graphic.  Any student who has lost a friend or a family member in an auto accident may not want to view the video.  If your child does not want to view the video or you would like to preview it, please contact our office.
