We are pleased you have chosen Mid-America Driver Training for the Adult Remedial Program. This program is a requirement for a two point credit on your driving record, a twelve point suspension or an under-age alcohol offense. You may be asked to complete this course due to a court order or as a requirement by an employer.
Because we offer this program once per month, pre-registration is required. Your completed registration form and non-refundable payment of $100.00 is due one week prior to the scheduled class date. Payment may be made by cash, personal check, money order or MasterCard/Visa. Failure to do so will forfeit your participation.
Also plan to bring a form of identification to be copied for our records. (This can be a social security card, court papers or state ID)
Mid-America Driver Training reserves the right to cancel. You will be notified by phone if your class has been postponed. All payments intended for the original date will be applied to the next scheduled class.
Your session is scheduled for Saturday, ____________200__. The class is held from 8:00 a.m. until 4:00 p.m. Please bring a sack lunch or money to order out. You will have a working lunch.
